District Nurses

District nurses in the Waimarino district provide a comprehensive range of quality, patient-focused, professional and holistic nursing services that are primarily home based. They provide short term health services in the community on discharge from hospital or as referred by a health professional:

 

  • skin and wound care
  • supplies – ostomy dressings or continence appliances
  • assessments for short-term services – home help, Meals on Wheels, personal care
  • access to equipment and aids on short loan – commodes, toilet seats, shower stools
  • monitoring and supervision of patients with complex needs and chronic health problems
  • patient/carer education
  • access to specialist nurse services – palliative, diabetes, respiratory, stoma therapy, continence
  • referrals to other support services – social worker, occupational therapist, mental health team, physiotherapist, Access Ability.

 
District nurses address health care needs that cannot be met by generalist medical or nursing services alone. Without this advanced nursing care, the patient could be at risk of further health deterioration.
 
District nurses use specialised nursing knowledge, assessment skills, professional judgement and have a knowledge of community resources. They collaborate with other health professionals and work in partnership with people through the lifespan in their own home, community or hospital setting. The nurses function independently and interdependently in assessing needs, planning, delivering and evaluating care.
 
Access

  • Rural nurses can be accessed through referral from your doctor, the hospital or Iwi health provider.
  • Some patients may be outside the rural nurse admission criteria, if so, we will refer on to the appropriate service. We can visit patients in their homes, workplace, at the Waimarino Rural Health Centre, a Marae or school.
  • We will work in partnership with the patient, family/whanau and other providers such as the General Practitioner or Iwi provider to plan and co-ordinate the necessary care.

Social Work Services


The social worker works with patients who live with physical health and disability issues by providing holistic and culturally-safe services which enhances total health care. The social workers service is professional, timely and confidential and they will see clients in their own homes or at the centre.

If you are a Waimarino patient requiring transport to/from Whanganui Hospital phone through by 3pm the day prior to appointment. You will be contacted with confirmation of departure time that evening.
